What do we do if our weight relapsed?

We hear a lot about cases that have done bariatric surgeries but didn’t get the desired result, whether it was weight loss or getting rid of certain diseases that were caused by obesity.

And in some cases, after surgery, they gain back the weight and suffer  from health complications

What is the reason behind the failure of bariatric surgeries?  and do you think that revisional surgeries are the main solution to all cases of weight relapse and regain?

In this article, we will talk about these Revisional surgeries; who are the candidates for them, and how to prevent the initial surgery from failure.

What are Revisional surgeries?

The ASMBS categorizes reoperation into three categories: conversion, corrective, or reversal.

How is each type of revisional surgery used for each relapse case?

  • Conversions are used for complications and inadequate response to the primary surgery
  • Corrective procedures are used when primary surgeries have either poor construction or complication.
  • Reversal procedures are needed when the primary surgery has caused an intolerance: such as malnutrition.

Why do we gain weight after the surgery? Why does the initial surgery fail?

There are many reasons that cause weight to be regained and increase after surgery, including:

  • Failure to comply with post-operative follow-ups, whether nutritional or psychological follow-ups
  • Lack of commitment to unhealthy eating regimens such as resorting to unhealthy food that has large calories, like Sugars, sweets, soft drinks, crackers, and not large quantities of food

Is anyone gaining weight a candidate for corrective operations?

Not all relapse cases are candidates for the revisional surgeries

Because we can lose this regained weight by following up on follow-ups and adhering to a customized eating regimen and a healthy lifestyle

But there are cases that require the necessity of performing these surgeries, including:

Not losing weight despite adhering to the customized regimen for it, and also unjustified weight gain

Having stomach complications

The presence of unlikely symptoms of GERD

If we have these symptoms, we must do a stereoscopic CT scan of the stomach to make sure that there is a failure in the initial surgery and therefore we need to do a revisional surgery

How do we know that the initial surgery failed? Is there any other indicator than weight regain?

If weight gain has caused metabolic diseases that include high blood pressure, high blood sugar, increased body fat, and abnormal levels of cholesterol or triglycerides.

What do we do to prevent the initial surgery from failing in the first place?

  • Choosing the right Medical team and choosing an experienced surgeon to know what type of surgery is appropriate for each case
  • Adherence to nutritional and behavioral follow-up after surgery
  • Stick to diet and exercise after surgery

Are all bariatric centers capable of performing these surgeries?

Not all bariatric centers, and not all doctors, are capable of this type of surgery because it requires an experienced surgeon and centers that use tools that are exclusively made specifically for this type of surgery.
